What's Next?
1. After the application and required forms (if any) are completed, enclose a check for the first premium (if paying by monthly bank draft, include 2 monthly premiums), payable to ACACIA LIFE INSURANCE COMPANY., and dated to match the date of the signatures on the application.*
2. Please provide a copy of owner's driver license / I.D. or passport or visa or alien registration card.
3. Mail the completed application, other required forms and the check for the initial premium to:

4. When we receive (via fax or mail) the completed application, required forms (if any) and check, we will automatically order the paramed exam.
